Indications :
Parenteral nutrition supplementation with long chain omega-3-fatty acids, especially eicosapentaenoic acid and docosahexaenoic acid, when oral or enteral nutrition is impossible, insufficient or contrain

Dosage :
Daily Dose: 1 up to max 2 mL Omegaven/kg body weight=0.1 up to max 0.2 g fish oil/kg body weight; =70 up to max 140 mL Omegaven for a patient with a body weight of 70 kg.

Maximum Infusion Rate: The infusion rate should not exceed 0.5 mL Omegaven/kg body weight/hr corresponding to 0.05 g fish oil/kg body weight/hr.

The maximum infusion rate should be strictly adhered to, otherwise a severe increase in the serum triglyceride concentration can be observed. Omegaven should be administered simultaneously with other fat emulsions. On the basis of a recommended total daily lipid intake of 1-2 g/kg body weight, the fish oil portion from Omegaven should constitute 10-20% of this intake. Administration: For infusion via central or peripheral vein. Shake before use. When Omegaven is to be administered with other infusion solutions (eg, amino acid or carbohydrate solutions) via a common infusion line (by-pass, y-tube), the compatibility of the solutions/emulsions used must be ensured.

Duration : The duration of administration should not exceed 4 weeks.

Each 100 mL of emulsion for infusion contains: Highly refined fish oil 10 g containing: Eicosapentaenoic acid (EPA) 1.25-2.82 g, docosahexaenoic acid (DHA) 1.44-3.09 g, myristic acid 0.1-0.6 g, palmitic acid 0.25-1 g, palmitoleic acid 0.3-0.9 g, stearic acid 0.05-0.2 g, oleic acid 0.6-1.3 g, linoleic acid 0.1-0.7 g, linolenic acid ≤0.2 g, octadecatetraenoic acid 0.05-0.65 g, eicosaenoic acid 0.05-0.3 g, arachidonic acid 0.1-0.4 g, docosaenoic acid ≤0.15 g, docosapentaenoic acid 0.15-0.45 g, dl-α-tocopherol (as antioxidant) 0.015-0.0296 g; glycerol 2.5 g; purified egg phosphatide 1.2 g.

Product Information Name Of Medicine Intralipid ® 10%, 20% and 30% (Sterile fat emulsions containing soya oil, egg lecithin and glycerol). Pharmacology The particle size is less than 1 micron which gives a stable emulsion and excludes the risk of fat embolism. Intralipid enters the bloodstream in a similar manner to natural chylomicron-rich lymph in both size and form, and is eliminated from the circulation according to the same kinetic principles as dietary chylomicron-rich lymph. It is formulated as a concentrated source of energy to be used together with carbohydrates and amino acids in parenteral nutrition, is isotonic, and provides a source of basal phosphate requirements (15 mmol/L) and a source of vitamin E. The vitamin E content in Intralipid 10%, 20% and 30% is 6.5, 13 and 19.5 microgram/mL, respectively. Intralipid prevents Essential Fatty Acid Deficiency (EFAD) and corrects the clinical manifestations of EFAD. However, for patients requiring complete parenteral nutrition, complementary vitamin supplements are required. Pharmacokinetics Intralipid is eliminated from the circulation via a pathway similar to that of endogenous chylomicrons, at least early on in the catabolism. The exogenous fat particle is taken up by the LDL receptors and primarily hydrolysed in the peripheral circulation which removes the triglycerides. Thus a transient triglyceride level may be elevated after Intralipid infusion. The triglycerides are hydrolysed by lipoprotein lipase to fatty acids and glycerol. The free fatty acids are used by the muscle as an immediate source of energy or re-converted into triglycerides and stored as a fat in the subcutaneous tissue for energy reserve. The free fatty acids also undergo entero-hepatic recycling and re-enter the systemic circulation in the form of Very Low-Density Lipoproteins (VLDL). Both the elimination and the oxidation rates are dependent on the patient’s clinical condition; elimination is faster and utilisation is increased in postoperative patients andin trauma, while patients with renal failure and hypertriglyceridaemia show lower utilisation of exogenous fat emulsions. There is no information available on the elimination half-life.
